Enroll Online
Once you decide which insurance carrier and plan option you would like to enroll on, you will need to complete the insurance carrier’s application as well as (in most instances) a health questionnaire. Lifewise, Regence, and Asuris NW allow you to enroll online. You can even complete the required health questionnaire online.

You can also enroll by mail. You will need to print off the carrier’s application, as well as the health questionnaire (see “Get More Information” below to see if you qualify for a Health Questionnaire waiver).

OPTIONS FOR THOSE 65 AND OLDER
For residents of Washington State (you must live here 50% or more each year), we recommend the Regence MedAdvantage plans in Western Washington and the Asuris TruAdvantage plans in Eastern Washington.  These plans provide coverage over and above your Medicare A and B programs, at an additional cost.

Provider Search
